Proactive Home Health Care Services, Inc. is a Medicaid provider through the Community Care Service Program (CCSP) and the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) under the New Option Waiver Program (NOW) and The Comprehensive Supports Waiver Program (COMP).

Proactive Home Health Care Services provide services to the following Georgia counties: Bibb, Baldwin, Hancock, Jones, Monroe, Putnam, Twiggs, Washington, Wilkinson, Laurens, Crawford, Houston, Peach, Bleckley, Johnson, Treutlen, Glascock, Jefferson, Warren, Morgan, Greene, Newton and Jasper.

Our services through the Community Care Services Program (CCSP) provides personal support in the comfort of your own home.

Personal Supports Services: Providing assistance with Activities of Daily Living (ADLs) such as feeding, meal preparations, bathing, dressing, personal hygiene, grooming, shaving, toileting, transferring/mobility, and use of incontinence products. Also providing support in the area of safety

issues especially in the increase risks of injuries from falls.

Extended Personal Support: Providing services for tasks designed for those who need assistance with daily living activities, as well as relieving those who normally provide care.

Instrumental Activities of Daily Living (IADLs): Providing assistance with household management such as light housekeeping (sweeping, vacuuming, mopping, changing of bed linen, laundry, ironing, transportation to medical appointments, pharmacy, and grocery shopping), monitoring and encouraging medication compliance, encourage physical activities, community outings, and social activities.

NOW/COMP Medicaid Waiver Program: Providing assistance to families with individualized support services needed through Community Living Supports {(CLS) services that are individually tailored supports that assist with the acquisition, retention, or improvement in skills related to a participant's continued residence in his or her family home} and Community Access Individual {(CIA) services that are designed to assist the participant in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active participation and independent functioning outside the participant's place of residence}.
